Hi, everyone! We JUST found out about this opportunity a couple weeks ago and my five sisters and our adult children are scrambling to get everything together! This forum is ALL THE THINGS! So helpful... thanks to everyone! I have a few questions I haven’t found answered yet. My cousin and his daughter applied back in July. I’m following the model of their docs but because they haven’t received their certificates of lineage yet, I’m a little concerned about correct format for the application. They just wrote a cover letter stating they wish to apply and listing the docs enclosed. Does something like this suffice? Or is there an actual application form/format? I am seeing on the Lux website we need something called a "model biographical questionnaire" (see mj.public.lu/nationalite/), but I can't figure out what this is. Any help would be much appreciated!!

Welcome, For the 'Phase 1', the request for the certificate of Lineage (d'aieul), you NEED; The birth, marriage, death certificate of your lux ancestor from 1900 the birth certificate of all descendants including yourself. The marriage certificates of any women, including yourself. A certified copy of your passport. What people can include that might help; Any other certificates between yourself and your ancestor that aren't absolutely necessary. A cover letter. A small Family tree showing the lineage
The cover letter can be written in English basically stating that you are applying for the recovery of citizenship, included are all the required documents, etc ,etc. Include address and date. Another question: We have family members all making one application together who are living in 5 US states. Do we have to (originally sign) the application letter? That sounds like a lot of mailing one document around.

I'm not sure what you mean by application letter, if you mean the cover letter, the one i sent is really designed to be completed by one person not a group. It would depend if you are submitting all your applications in as one big application (probably recommended), or individually. For the cover letter, i would fill it out for each person including their name, print it out and then put p.p next to their printed name and sign it yourself; en.wikipedia.org/wiki/ProcurationSince the cover letter is an optional document you don't need to have everyone sign it, one person to sign it on the others behalf is sufficient. Luxembourg is really good for efficient government, they aren't crazy about all the dotted i's and crossed t's, as long as you have the essential info they seem pretty cool about the small stuff.
