Questionnaire, English Translation

The questionnaire doesn't actually tell you what you need for phase 2. It's just a short amount of questions about yourself that you fill out ie: first name, last name, if you have kids (if you do, then their names), if you're married, what your nationality is, etc. the ministry website I had a link to an English version that was to be used for translation only but if I remember correctly the link doesn't work anymore. Regardless the questionnaire is supposed to be in French and German. So you can go to the ministries website and download a pdf file of the questionnaire and just use something like google translator to figure out what the questions are. I have asked one of the workers for the ministry of justice if the answers to the questions had to be in French or German or if I could simply write them in English and the told me responding to the questions in English is ok.

Moike, thanks for the info. I was just wanting to see the english translation so that I could have a look at what types of questions were on it. I already have a separate list of everything that is needed for phase 2, just wanting to make sure I have a complete understanding of the questionnaire. The bit on English answers is interesting, everything I have read from others says that they translated all their answers into French or German. If they can be in English then that would be very helpful and save a bit of cash on certified translation.

Helena, we do have both the original and the amended birth certificates, we were just waiting on the courts to release the adoption decree to link the two together because my mom's name wasn't on the original birth certificate, only her last name because she was adopted a few days after birth. Thanks for your info too, that will be a huge help in assembling everything.
